Session 1: How to Transform Patient Care by Integrating AI

Computerization of healthcare documentation removed much of the intimacy in providing care. Doctors are spending most of their time tethered to a computer, typing notes into an EMR, which is driving increased rates of burnout and patient dissatisfaction, and a lack of engagement. Luckily, healthcare technology is rapidly evolving. One transformative approach involves the combination of conversational, ambient, and generative AI, to remove the computer barrier between physician and patient. This combination holds great potential to revolutionize the way healthcare professionals interact with patients, streamline workflows, and ultimately deliver personalized, efficient, and compassionate care.

Session 2: Strategies for Success in Value-Based Care

As health systems and provider groups continue to adopt value-based care (VBC), their financial risk options are becoming more complex. In this session, attendees will learn how Duly Health and Care (formerly DuPage Medical Group) evolved its VBC model to deliver lower-cost, higher-quality care. To achieve continued VBC success, open communication between physicians and their participation in clinical decision-making is critical. This session will highlight Duly’s VBC journey, including evaluating the many VBC program options, an overview of the participation decision-making process, and degrees of provider risk-sharing tolerance. This session will specifically cover Duly’s evaluation and decision-making process in their recent move to participate in the new CMMI ACO Reach program. The speakers will share insights to date and tools used to help make care navigation easier for patients and their care teams. While this VBC model is specific to senior patients, Duly engages in VBC contracts across its entire patient population, with approximately 30,000 patients in a risk-bearing arrangement.
